Heat Exposure and Longevity: Can Turning Up the Temperature Help You Live Longer?
For thousands of years, humans have deliberately exposed themselves to heat. From Finnish saunas and Turkish hammams to Japanese onsen and Native American sweat lodges, heat has long been associated with health, recovery and wellbeing.
In recent years, however, heat therapy has prompted intriguing questions → what is the science behind heat therapy and could regular heat exposure actually help us live longer?
Early evidence suggests the answer is promising, but nuanced. While a growing body of research suggests that regular sauna bathing and passive heat exposure may reduce the risk of cardiovascular disease, improve metabolic health and even lower all-cause mortality, the evidence is strongest for correlation rather than causation.
We also need more information on how to use heat therapy safely. Heat is a physiological stressor, and like exercise, its benefits depend on the dose. Too little may have little effect; too much can become dangerous.
The Science of Hormesis: Why Stress Can Be Good
One of the key concepts behind heat therapy is hormesis.
Hormesis describes the phenomenon whereby a mild, controlled stress stimulates the body to become stronger and more resilient. Exercise is perhaps the best-known example → lifting weights damages muscle fibres, but the repair process leaves muscles larger and stronger.
Heat appears to work in a similar fashion.
When core body temperature rises by around 1–2°C, the body responds by activating numerous protective pathways, including:

These responses are thought to improve cellular resilience and may help slow aspects of biological ageing, although exactly how much they contribute to human longevity remains an active area of research.
Exploring The Finnish Sauna Studies
Much of the excitement surrounding heat exposure comes from a series of large studies conducted in Finland, where sauna bathing is deeply embedded in everyday life.
One landmark study followed 2,315 middle-aged Finnish men for over 20 years.
Compared with men using a sauna once a week:
Those using a sauna 2–3 times per week had a 22% lower risk of sudden cardiac death.
Those using a sauna 4–7 times per week had a remarkable 63% lower risk of sudden cardiac death.
Frequent users also experienced approximately 40% lower all-cause mortality and significantly lower rates of fatal cardiovascular disease.
Importantly, these are observational studies. They demonstrate a strong association but cannot prove that sauna use alone caused the reduction in mortality. For instance, frequent sauna users may also engage in healthier lifestyles overall.
Nevertheless, even after adjusting for smoking, alcohol intake, body weight, exercise and socioeconomic status, the associations remained impressive.
Why Might Heat Protect the Heart?
During a sauna session, heart rate commonly rises to 100–150 beats per minute, comparable to moderate aerobic exercise.
Meanwhile:
Blood vessels dilate.
Blood flow to the skin increases dramatically.
Cardiac output rises.
Blood pressure often falls after the session.
Repeated exposure appears to improve the function of the inner lining of blood vessels. The health of blood vessels is one of the strongest predictors of cardiovascular disease.
Several studies also demonstrate modest reductions in resting blood pressure among regular sauna users, typically in the region of 5–10 mmHg systolic, an effect comparable with many lifestyle interventions.
Benefits Beyond the Heart
Regular heat exposure may also influence other body systems including metabolic health, recovery and brain health.
Heat exposure increases blood flow to skeletal muscle and may improve glucose uptake, producing some physiological effects that overlap with moderate exercise. Whilst it should not be considered a replacement for physical activity, several studies have reported improvements in:
insulin sensitivity
glucose regulation
vascular function
inflammatory markers
Athletes have used saunas for decades. The evidence for performance enhancement remains mixed, and recent systematic reviews conclude that higher quality trials are still needed. However, we have more evidence that suggests heat exposure may:
reduce muscle soreness
improve flexibility
increase plasma volume
support endurance adaptations
And perhaps the most fascinating observations involve neurological disease.
Some observational studies have found that frequent sauna users have substantially lower rates of dementia and Alzheimer's disease. This may be due to a mix of improved cerebral blood flow, reduced vascular disease and lower chronic inflammation.
However, these findings remain strictly observational and should be interpreted cautiously until confirmed by more robust studies.
Heat Is Not Always Beneficial
While controlled heat exposure may improve health, uncontrolled heat exposure is a very different situation.
Environmental heatwaves increase mortality worldwide, particularly among:
older adults
infants
pregnant women
individuals with cardiovascular disease
those taking medications that impair sweating or hydration
This highlights the important distinction that intentional heat exposure is brief, controlled and followed by cooling and rehydration, whereas heat illness is prolonged, uncontrolled and often accompanied by dehydration.
Although generally safe for healthy adults, sauna use is not appropriate for everyone. Extra caution is advised for people with:
unstable angina
uncontrolled hypertension
severe aortic stenosis
recent heart attack
acute illness with fever
severe dehydration
Alcohol significantly increases the risk of dehydration, low blood pressure and accidents during sauna use and should be avoided before and during heat exposure.
Pregnant women and individuals with chronic medical conditions should seek personalised medical advice before beginning regular sauna use.
How Much Heat Is Enough?
There is no universally accepted "optimal dose."
However, the strongest observational evidence comes from people using traditional dry saunas:
4–7 sessions per week
15–20 minutes per session
temperatures of approximately 70–100°C
Beginners should start with much shorter sessions and gradually increase exposure while maintaining good hydration.
The goal is adaptation, not endurance.
Feeling dizzy, nauseated or confused is not evidence of a better workout. It is a sign to stop immediately.

Heat exposure is emerging as one of the most intriguing lifestyle interventions in longevity science.
The evidence suggests that regular sauna use may improve cardiovascular health, lower blood pressure, enhance metabolic function and is associated with a lower risk of premature death. The underlying biology is plausible, involving heat shock proteins, improved vascular function and beneficial cellular stress responses.
However, enthusiasm should be balanced with scientific caution. Much of the strongest evidence remains observational, and sauna bathing is best viewed as one component of a healthy lifestyle rather than a miracle intervention.
Exercise, sleep, nutrition, smoking cessation and blood pressure control still have far stronger evidence for extending both lifespan and healthspan.
When used correctly, heat therapy may prove to be another valuable tool in helping us not simply live longer, but age better.
